Wegovy Dosing Informationn

The starting Wegovy dose is 0.25 mg once a week for four weeks, and then your Wegovy dose will increase every 4 weeks until you reach a maintenance dose of 1.7 mg or 2.4 mg weekly. The Wegovy dosing schedule starts at a low dose and increases over months to reduce the risk of gastrointestinal side effects. If patients do not tolerate a dose during dosage escalation, consider delaying dosage escalation for 4 weeks.

  • What is the medicine?
    • This is a medicine for weight loss and weight maintenance that contains the active ingredient semaglutide. It is similar to the natural hormone glucagon-like peptide-1 (GLP-1) released from the gut after a meal. It acts on the targets (receptors) in the brain that control appetite, causing you to feel increased satiety, reduced hunger, and fewer cravings. This will help you eat less and reduce your body weight. The medicine can also help prevent heart disease.
  • What is the preparation used for?
    • It is used in conjunction with diet and physical activity for weight loss and as an aid to weight maintenance. It is used in adults with
      • a BMI of 30 kg/m2 or more (obesity) or
      • a BMI of at least 27 kg/m2, but less than 30 kg/m2 (overweight) who have weight-related health problems (such as diabetes, high blood pressure, abnormal blood lipid levels, breathing problems during sleep, so-called "obstructive sleep apnea," or a history of heart attack, stroke, or blood vessel problems). BMI (Body Mass Index) is a measure of your body weight in relation to your height. This medicine is used together with diet and physical activity for weight control in adolescents aged 12 years or older with
      • Adipocytes and
      • Body weight > 60 kg.
    • As a young patient, you should only continue treatment with this medicine if you have lost at least 5% of your body mass index (BMI) after using the 2.4 mg dose or the maximum tolerated dose for 12 weeks. Consult your doctor before continuing treatment.
  • The medicine must not be used
    • if you are allergic to semaglutide or any of the other ingredients of this medicine.
  • Like all medicines, this medicine can cause side effects, although not everybody gets them.
  • Serious side effects
    • Common (may affect up to 1 in 10 people)
      • Complications of diabetic eye disease (diabetic retinopathy). If you have diabetes, you should inform your doctor if you experience any eye problems, such as visual disturbances, during treatment with this medicine.
    • Uncommon (may affect up to 1 in 100 people)
      • Inflamed pancreas (acute pancreatitis). Signs of pancreatitis may include severe and long-lasting pain in your abdomen, which may radiate to your back. Consult your doctor immediately if you experience any of these symptoms.
    • Rare (may affect up to 1 in 1,000 people)
      • Severe allergic reactions (anaphylactic reactions, angioedema). You should seek immediate medical help and tell your doctor immediately if symptoms such as: difficulty breathing, swelling, dizziness, rapid heartbeat, sweating, and loss of consciousness, or rapid swelling under the skin in areas such as the face, throat, arms, and legs occur, which can be life-threatening if the swelling in the throat blocks the airways.
    • Not known (frequency cannot be estimated from the available data)
      • Intestinal obstruction. A severe form of constipation with additional symptoms such as abdominal pain, bloating, vomiting, etc.

      • Change in the taste of food or drinks
      • Hypoglycemia in patients with type 2 diabetes.
        • The warning signs of hypoglycemia may occur suddenly. They may include: cold sweat, cool pale skin, headache, rapid heartbeat, nausea or feeling very hungry, blurred vision, tiredness or weakness, nervousness, anxiety or confusion, difficulty concentrating or tremors.
        • Your doctor will tell you how to treat hypoglycemia and what to do if you notice these warning signs.
        • Hypoglycemia is more likely to occur if you are also taking a sulfonylurea or insulin. Your doctor may reduce your dose of these medicines before you start using this medicine.
    • Uncommon (may affect up to 1 in 100 people)
      • Low blood pressure
      • Dizziness or lightheadedness when standing up or sitting up due to a drop in blood pressure
      • Raised heart rate
      • Increased levels of pancreatic enzymes (such as lipase and amylase) detected in blood tests
      • Delayed gastric emptying.
  • Reporting of side effects
    • If you experience any side effects, contact your doctor, pharmacist or nurse. This also applies to side effects not listed.

    • The use of this medicine is not recommended if you:
      • are using other weight-loss products,
      • have type 1 diabetes,
      • have severely impaired kidney function,
      • have severely impaired liver function,
      • have severe heart failure,
      • have diabetic eye disease (retinopathy).
    • There is little experience with this medicine in patients who:
      • are 85 years of age or older,
      • have liver problems,
      • have severe stomach or bowel problems leading to delayed gastric emptying (so-called gastroparesis), or have inflammatory bowel disease.
    • Please consult your doctor if any of the above applies to you.
    • Dehydration
      • During treatment with this medicine, you may experience nausea, vomiting, or diarrhea. These side effects may lead to dehydration (fluid loss). It is important that you drink enough to prevent dehydration. This is especially important if you have kidney problems. Talk to your doctor if you have any questions or concerns.
    • Inflammation of the pancreas
      • If severe and persistent abdominal pain occurs, seek immediate medical attention, as this may be a sign of inflammation of the pancreas (acute pancreatitis).
    • Patients with type 2 diabetes
      • This medicine cannot be used as a substitute for insulin. Do not use this medicine in combination with other medicines containing GLP-1 receptor agonists (such as liraglutide, dulaglutide, exenatide, or lixisenatide).
    • Hypoglycemia
      • The combination of this medicine with a sulfonylurea or insulin could increase the risk of hypoglycemia. For more information about the warning signs of hypoglycemia, see the "Side Effects" category. Your doctor may ask you to measure your blood sugar level. This will help your doctor decide whether the dose of the sulfonylurea or insulin needs to be changed to reduce the risk of hypoglycemia.
    • Diabetic eye disease (retinopathy)
      • If you have diabetic eye disease and use insulin, this medicine may cause a worsening of your vision, which may require treatment. Rapid improvements in blood sugar control may cause a temporary worsening of diabetic eye disease. If you have diabetic eye disease and experience eye problems while using this medicine, talk to your doctor.
    • Children and adolescents
      • The safety and effectiveness of this medicine in children under 12 years of age have not been studied, and use is not recommended in this age group.

 

  • Driving and using machines
    • This medicine is unlikely to affect your ability to drive and use machines. Some patients may experience dizziness when using this medicine, especially during the first 4 months of treatment. If you feel dizzy, take extra care when driving or operating machinery. If you need more information, talk to your doctor, pharmacist, or nurse.
    • Patients with type 2 diabetes
      • If you use this medicine in combination with a sulfonylurea or insulin, low blood sugar (hypoglycemia) may occur, which may impair your ability to concentrate. Avoid driving or operating machinery if you experience any signs of low blood sugar. See the "Patient Information" section for information on the increased risk of hypoglycemia and the "Side Effect" section for warning signs of hypoglycemia. Consult your doctor for further information.
  • Pregnancy
    • This medicine should not be used during pregnancy, as it is not known whether it can harm your unborn child. For this reason, it is recommended that you use contraception while using this medicine. If you want to become pregnant, you must stop taking this medicine at least two months beforehand. If you become pregnant, are pregnant, think you may be pregnant, or are planning to become pregnant, tell your doctor immediately before using this medicine, as your treatment must be stopped.
  • Breast-feeding
    • Do not use this medicine if you are breastfeeding, as it is not known whether it passes into breast milk.
  • How the medicine is used
    • The medicine is given as an injection under the skin (subcutaneous injection). Do not inject the medicine into a vein or muscle.
      • The best places for injection are the front of the upper arm, the thighs, or the abdomen.
      • Before using the pen for the first time, your doctor, pharmacist, or nurse will show you how to use it correctly.

  • When to use the medicine
    • You should use this medicine once a week, if possible always on the same day of the week.
    • You can give yourself the injection at any time of day, regardless of meals.
    • If necessary, you can change the day of the week of your weekly injection of this medicine, as long as at least 3 days have passed since your last injection. Resume the once-weekly dosing after selecting a new day of use.
  • Use with other medicines
    • Tell your doctor, pharmacist, or nurse if you are taking, have recently taken, or intend to take any other medicines.
    • In particular, tell your doctor, pharmacist, or nurse if you are taking any medicines containing:
      • Warfarin or other similar medicines taken by mouth to reduce blood clotting (oral anticoagulants). Frequent blood tests to determine your blood's ability to clot may be necessary if you start treatment with, for example, warfarin or similar medicines.
